Register By Mail Or In-Person
To register by mail or drop-off, please complete the registration form. We do not accept phone registrations.

Concussion Law
Due to the increase in concern for concussions in youth sports, there is now a Wisconsin Concussion Law. Our department requires all participants to review the              
Concussion Agreement and Information
prior to registering for programs. Our participant safety is our number one concern!

Credit Cards
We accept Visa and Mastercard online and in-house. However, credit card payments require a transaction fee through the processing company and are non-refundable.

Inclement Weather
Call our HOTLINE at 608-821-8357, visit the Public Lands, Recreation, and Forestry page, or Facebook to see the status of a program due to inclement weather. Any programs postponed due to inclement weather will not be rescheduled. Programs canceled due to facility unavailability will not be rescheduled. When the Middleton-Cross Plains School District closes the schools, all recreation programs are cancelled.

Min/Max Enrollment
Middleton Recreation will determine the minimum number of participants needed to hold the program, and reserves the right to cap enrollment when deemed necessary.

Confirmation of Enrollment
Once you have registered, plan on attending at the listed times and dates. Please feel free to call to verify enrollment and/or ask questions. The Recreation Division will only contact you if the program is full, canceled, or there are changes.

Age/Grade Requirements
All youth program age restrictions will be based on the first day of the program. All grade restrictions will be based on the upcoming 2012-2013 school year. Age and grade requirements are set to benefit all children and to make instruction easier for the program leader. All participants must register for the correct class or they may be contacted to move classes or lose their spot in the program.

For program enrollment, Residents are those that reside within the Middleton-Cross Plains School District boundaries. Non-Residents are those that are not part of the district. For the Aquatic Center and facility rentals, Residents are those that reside within the City boundaries (this does not include the Town of Middleton).

Photo Policy
Middleton Recreation Division programs and events are often photographed for promotional purposes. If you do not wish to be included in a Recreation Division photo, please indicate this to the instructor and/or photographer.

Participants must assume full responsibility for injuries while participating.
