Case Management

The Middleton Senior Center has two Case Managers on staff who are
available to answer questions and refer seniors, or their families and
caregivers, to appropriate services available in the community. To

Information and Referral

Case Managers are a good resource for seniors, their families and
caregivers if they have questions about:

• Medicare, Medigap, Medicaid and other medical assistance.
• Long-term care.
• Disability & Social Security.
• Food stamps, energy services and rental assistance.
• Power of Attorney, Guardianship and referrals to benefit
  specialists.

Case Managers can connect seniors to programs such as:

• Home delivered meals or other meal options.
• RSVP – transportation to medical appointments.
• Supportive home care.
• Support groups.
• Roadside Review—safe driving options for seniors.
